Konzepte zur Unterstützung dynamischer Aktualisierungsprozesse von Leitlinien und ihre Implementierung in der Praxis – systematische Literaturrecherche

Zusammenfassung

Hintergrund

Klinische Leitlinien, die entscheidend für eine evidenzbasierte Gesundheitsversorgung sind, sollten stets auf dem neuesten Stand gehalten werden. Ein Aktualisierungsprozess, der sowohl für die Leitlinienentwickler als auch für die Nutzer praktikabel ist, stellt jedoch eine Herausforderung dar.

Ziel der Arbeit

Diese Arbeit gibt einen Überblick über die derzeit diskutierten methodischen Ansätze zur dynamischen Aktualisierung von Leitlinien und systematischen Reviews.

Material und Methoden

Im Rahmen eines Scoping Reviews wurde eine Literaturrecherche in den Datenbanken MEDLINE, EMBASE (via Ovid), Scopus, Epistemonikos, medRxiv sowie in Studien- und Leitlinienregistern durchgeführt. Eingeschlossen wurden deutsch- und englischsprachige Konzepte zur dynamischen Aktualisierung von Leitlinien und systematischen Reviews sowie dynamisch aktualisierte Leitlinien und systematische Reviews bzw. deren Protokolle.

Ergebnisse

Die in den eingeschlossenen Publikationen am häufigsten berichteten wesentlichen Prozessschritte, die im Rahmen einer dynamischen Aktualisierung angepasst werden müssen, sind die (1) Etablierung kontinuierlich arbeitender Leitliniengruppen, (2) die Vernetzung von Leitlinien untereinander, (3) die Aufstellung und Anwendung von Priorisierungskriterien, (4) eine Anpassung der systematischen Literatursuche sowie die (5) Verwendung von Tools zur Effizienzsteigerung und die Digitalisierung der Leitlinie.

Diskussion

Die Umstellung auf Living Guidelines erfordert einen veränderten zeitlichen, personellen und strukturellen Ressourcenbedarf. Die Digitalisierung von Leitlinien und der Softwareeinsatz zur Effizienzsteigerung sind notwendige Instrumente, garantieren allein aber nicht die Umsetzung lebendiger Leitlinien. Erforderlich ist ein Prozess, in den auch die Disseminierung und Implementierung integriert werden müssen. Nach wie vor fehlen standardisierte Best-Practice-Empfehlungen zum Aktualisierungsprozess.
