Therapiestrategien beim Rektumkarzinomrezidiv

Zusammenfassung

Multimodale Therapieansätze mit neoadjuvanter Strahlen- und Chemotherapie gefolgt von onkologischer und totaler mesorektaler Resektion (TME) haben die Rezidivrate selbst bei lokal fortgeschrittenen Rektumkarzinomen deutlich reduziert. Dennoch entwickeln bis zu 10 % der Patienten ein Rezidiv. Die chirurgische R0-Resektion stellt bei der Behandlung eines lokal rezidivierten Rektumkarzinoms (LRRC) die einzige Chance auf Heilung dar. Aufgrund der durch Vorbehandlungen bzw. -operationen veränderten Anatomie und Physiologie des kleinen Beckens sowie der Lokalisation und Ausdehnung des Rezidivs erfolgt die Therapieentscheidung individualisiert und stellt weiterhin eine Herausforderung für das interdisziplinäre Team dar. Auch fortgeschrittene Tumoren mit Beteiligung benachbarter Strukturen können in ausgewiesenen Zentren durch multimodale Therapiekonzepte in potenziell kurativer Intention behandelt werden.
